Online Consumer Rights Request Form

Eliassen Group takes data privacy seriously. Under the privacy laws of California, Virginia, Colorado, Utah, and Connecticut, you may have the right to:

  • Request the categories of personal information that we've collected about you, the categories of sources from which we collected it, the purposes for collecting, selling, or sharing it, the categories of third parties to whom we disclosed it, and also the specific pieces of personal information that we’ve collected about you.
  • Request that we delete the personal information we have collected from you.
    • We use a two-step process for requests to delete. You will be asked to clearly submit your request to delete and to separately confirm your choice.
  • Request to correct inaccurate personal information that we’ve collected about you, subject to certain exceptions.1
  • Request to opt-out of profiling2 or the sale or sharing of your personal information for targeted advertisements.
    • Note: we do not sell personal information for monetary value.
    • If you have an account with us, we may ask you to sign-in to your account to submit this request.
  • Request to limit the use of your sensitive personal information.
    • If you have an account with us, we may ask you to sign-in to your account to submit this request.
  • Request a portable copy of your personal information.
  • Authorize an agent to make a request on your behalf. You can designate an authorized agent by mailing copies of two forms of identification to the address below that include your first name, last name, current address, phone number, and signed authorization letter allowing the agent to request on your behalf. We may verify this information by phone.
